Consent for Minor Child's Participation

I, , am the parent or legal guardian of the minor participant. I declare that I am well aware of and understand the nature of the Aikido Daiwa activities into which I place my child, and in executing this release and assumption of risk I expressly assume on my child’s behalf all normal and reasonable risks attendant to Aikido Daiwa martial arts activities. I expressly waive and release Birankai N.A. USA, its officers and instructors, Aikido Daiwa, its officers, instructors, managers, assigned staff, employees, and fellow students from any liability which may result from my child’s participation in Aikido Daiwa activities.
