RELEASE OF LIABILITY AND ASSUMPTION OF RISK

In consideration of my participation in any and all Aikido Daiwa activities, either on the premises of Aikido Daiwa or any other premises in the course of any Aikido Daiwa activities, I expressly waive and release Birankai N.A. USA, its officers and instructors, Aikido Daiwa, its officers, instructors, managers, assigned staff, employees, and fellow students from any liability which may result from my participation in Aikido Daiwa activities.

I further acknowledge that I am fully aware of and understand the nature of Aikido Daiwa activities in which I have chosen to participate, and the risks involved (including permanent disability and death), and that I hereby assume any and all risks associated with this type of martial arts activity. If I should become pregnant or injured, I agree that I will not continue in any aikido training until I have provided Aikido Daiwa with written approval from my attending physician.

I HAVE READ THE CONTENTS OF THIS RELEASE AND ASSUMPTION OF RISK, AND HEREBY REPRESENT THAT I UNDERSTAND THE MEANING AND LEGAL SIGNIFICANCE OF THIS RELEASE AND ASSUMPTION OF RISK.
